Eligibility & Requirements
1. Care recipient must be professionally diagnosed with Alzheimer's or related dementia.
3. Caregiver must be a family caregiver living in Dallas or Ellis County. Professional caregivers are not eligible.
4. A completed application must be submitted with a 60 second video OR 150 words sharing why the family caregiver in your life should receive a holiday gift basket.
(Must meet all three requirements to qualify.)
